The “Report and Forms in the Patient Data Repository” document contains
- the description of the technical structure of the CDA R2 document in the general part and the CDA R2 structure of the report used in Finland
- the descriptions in CDA R2 format of more detailed data content, such as diagnoses, risks, procedures, and physiological measurements
- general structures for forms in CDA R2 format
- comments on the CDA R2 body section of the service event document.
In addition to the “Report and Forms” documents, there are separate, more detailed data-content-specific specifications, which can be found as their own documents.
The “Report and Forms” document focuses mainly on documenting the body sections of the CDA R2 document. The header part of the CDA R2 document and its information (description information) are described in separate documentation Kanta – eArchive CDA R2 Header Guidelines.
The “Report and Forms” specifications are divided into different branches and these branches are not fully forward and backward compatible:
- The 2023 branch corresponds to the more recent data contents of the THL Code Service; no changes were made to the actual clinical data in the branch. In the branch, the clarifications and changes to the generation of the data of the person who made the entry have been taken into account.

The 2016 branch corresponds to the more recent data content of the THL Code Service.

The 2014 branch corresponds to the data contents specified in the THL Code Service in 2013 (partly from the old Core document).
